SMS Text Opt-In Terms & Conditions

Effective Date: August 3, 2023

You agree to receive recurring automated marketing and informational text (e.g., SMS and MMS) messages from Union One, including text messages that may be sent using an automatic telephone dialing system, to the mobile telephone number you provided when signing up or any other number that you designate. All the different types of messages you may receive (examples: informational messages, open enrollment reminder messages, billing messages, plan administration messages, etc) shall be known collectively as the “Programs.” Consent to receive automated marketing text messages is not a condition of any purchase.

Msg & Data rates may apply. Please consult with your carrier for rate information.

Message Frequency
Message frequency will vary. Unio One reserves the right to alter the frequency of messages sent at any time, so as to increase or decrease the total number of sent messages. Union One also reserves the right to change the phone number from which messages are sent.

Not all mobile devices or handsets may be supported and our messages may not be deliverable in all areas. Union One, its service providers and the mobile carriers supported by the program are not liable for delayed or undelivered messages.

Cancellation
If you do not wish to continue participating in any Program or no longer agree to these Terms, you agree to text the applicable Short Code (or regular long code phone number where applicable) or directly reply to any mobile message received from a Program with STOP, END, CANCEL, UNSUBSCRIBE, or QUIT in order to opt out of that Program at any time. You may receive an additional mobile message confirming your decision to opt out.

You understand and agree that the foregoing options are the only reasonable and exclusive methods of opting out. You also understand and agree that any other method of opting out, including, but not limited to, texting words or phrases other than those set forth above or verbally requesting Union One to remove you from our list, is not a reasonable means of opting out. You may be subscribed to multiple Programs across different Short Codes or regular long code phone numbers, and therefore you must separately text or reply STOP to each Short Code or long code phone number from which you wish to unsubscribe.

Support
For support regarding the Program, text “HELP” to the applicable Program’s Short Code or long code, or email us at info@unionone.com. Please note that the use of this email address, or texting “HELP” to the Program’s Short or long Code is not an acceptable method of opting out of the program. Opt outs must be submitted in accordance with the procedures set forth above.

Our Disclaimer of Warranty
The Programs are offered on an “as-is” basis and may not be available in all areas at all times and may not continue to work in the event of product, software, coverage or other changes made by your wireless carrier. We will not be liable for any delays or failures in the receipt of any mobile messages connected with any Program. Delivery of mobile messages is subject to effective transmission from your wireless service provider/network operator, and is outside of our control. We are not liable for delayed or undelivered mobile messages.

IMPORTANT INFORMATION ABOUT THIS PLAN

IMPORTANT: The monthly cost for coverage is based on your age at the start of the coverage and will increase on the policy anniversary date after you move into a new age bracket. This benefits guide is tailored for Members aged 18-79. If you depart from Teamsters, opt out of paying dues, or retire, you must notify the Teamsters VIP+ administrative office at (224) 770-5304. Not doing so within 90 days could delay or negate your eligibility for a refund.

Participation in this program is voluntary, and the decision to enroll rests solely with the Members. Members are responsible for bearing all associated costs. A $2 technology fee is included in all listed monthly costs for the following coverages: Short-Term Disability and Long-Term Disability. A $1 technology fee is included in all listed monthly costs for the following coverages: Member Life and Spouse Life.

This program is administered by Union One Benefits Administration.

Union One

This voluntary benefit plan is classified as a Safe Harbor plan and, as such, is not subject to the Employee Retirement Income Security Act of 1974 (ERISA). Teamsters does not contribute to the premiums for this plan on behalf of its Members, does not endorse the plan, and does not require Members to enroll in the plan.

FOR AD&D, STD & LTD: THIS IS AN EXCEPTED BENEFITS POLICY. IT PROVIDES COVERAGE ONLY FOR THE LIMITED BENEFITS OR SERVICES SPECIFIED IN THE POLICY.

For STD & LTD: These policies provide disability income insurance only and do NOT provide basic hospital, basic medical, or major medical insurance as defined by the New York State Department of Financial Services.

For Life: You have 30 days to notify Union One of your retirement if you wish to port or convert your Life Insurance.

North Carolina Residents: THIS IS NOT A MEDICARE SUPPLEMENT PLAN. If you are eligible for Medicare, review the Guide to Health Insurance for People with Medicare, which is available from the company.

Group Insurance coverages are issued by The Prudential Insurance Company of America, a Prudential Financial company, Newark, NJ. The Booklet-Certificate contains all details, including any policy exclusions, limitations, and restrictions, which may apply. Contract Series: 83500.
